Monday thru Thursday – 8:30 a.m. to 4:30 p.m.
Friday – 9:00 a.m. to 4:00 p.m.
The phones turn off from 11:30 a.m. to 1:00 p.m. for lunch
Providers are not in the office on Fridays
Closed National Holidays and Weekends

Insurance:
We are contracted providers for:
Medicare, DMBA, EMIA, PEHP, BlueCross & BlueSheild, United Healthcare, and most Select Health and Aetna plans

We are NOT providers for:
Medicaid, Benesight, Alternative Benefit Plans, Altius, CIGNA, Great West, IBEW, Molina, and most Tricare plans

If your insurance is not listed here, please contact our office to determine benefits. Patients with uncontracted insurances will be asked to pay our self pay price for services.

Payment Options:
Payment is expected at time of service.
We accept cash, checks, and Visa or Mastercard.

We are accepting new patients with pain problems by a referral only.
If you wish to be seen please have your primary care doctor fax us a referral.
If you are seeking help for substance abuse a referral is not required.
